About Us

Our office is located at 1600 N. Main Street in Wheaton, Illinois at the intersection of Main St. and Parkway. We are directly across the street from Walgreens.

Our mission at Chiropractic Medicine is to provide affordable, high-quality care to patients of all ages. We strive to not only correct a person's health problems, but to educate and enlighten people so that they do not encounter similar problems in the future.

This is accomplished with a thorough history and examination followed by the implimentation of a healthcare plan that is designed with the active participation of both the doctor and the patient.

Lifestyle modification may include a dietary evaluation and a home based program of physical exercises that are demonstated and taught at our clinic facility. We try to encourage the patient to be knowledgeable about their overall health and to engage actively in the process of recuperation which may assure a successful outcome with the least amount of cost in the shortest amount of time.

Dr. Forman received his pre-med education at Taylor University and his professional training at The National College of Chiropractic. He holds a B.S in Human Biology and a Doctorate in Chiropractic Medicine. He is one of only 3 DC's in the state of Illinois with Board Certification in Occupational Health.
